The index contains facility- and business-level public records only: certification, inspections, citations, penalties, staffing measures and ownership disclosures that CMS and state agencies publish. It contains no resident-level data and no protected health information. Names of individuals appear only where CMS publishes them as owners, officers or managers of a facility.
Report requests
When you request a report we store your delivery email, optional name, the facility, the scope you choose and payment status. Report content is private to you and is never added to the public index.
Saved facilities
Until accounts launch, saved facilities are kept in your browser’s local storage and never sent to us.
Analytics & cookies
No advertising trackers. Operational server logs are kept for security and debugging.
