Corrections & disputes

Two kinds of error

  1. Our error — a wrong link between records, a wrong county, a mislabeled field or a display bug. We fix these in the pipeline and the next build, and note the correction in the facility’s data-sources tab.
  2. A source error — a value that is wrong in the official CMS or state record itself. We do not edit official records. Facilities can dispute citations through informal dispute resolution (IDR / IIDR) with their state survey agency, and update ownership or enrollment data with CMS. Once the source changes, the next release flows through automatically.

How to report

Email the facility page URL (or CCN), the field that is wrong, and a link to evidence. Facilities and operators should include their role.

A corrections inbox and tracked dispute workflow will be added with accounts. Until then, open an issue on the project repository.
